QUALIFICATION REQUIREMENTS: Possess doctorate in dentistry from an accredited dental school approved by the Council on Dental Education of the American Dental Association (ADA) within the preceding 12 months, OR b) a doctorate from an accredited dental school approved by the Council on Dental Education of the ADA and have experience as a General Dentist of at least 12 months within the preceding 36 months.
Possess current, unrestricted license to practice dentistry in any one of the fifty States, the District of Columbia, the Commonwealth of Puerto Rico, Guam or the U.S. Virgin Islands and maintenance of same.
